At the Seattle Grace Hospital the day seems to start like any other but the situation quickly come to a head. Unexpectedly, as the main character, Meredith, and Bailey are taking care of a patient just arrived by ambulance, another oncoming ambulance bursts in the First Aid area and, dangerously swerving, hits violently the first one overturning it. The two paramedics in the first ambulance remain trapped upside down in the twisted metal. Immediately one of them, Stan, although conscious, seems in critical conditions. From the other ambulance comes out Shane, a young paramedic (played by our Gale) with sharp abdominal pain, and Mary, who was at the wheel, that apparently doesn’t suffer consequences from the crash. They’ll find out later that the incident has been caused by her epileptic attacks.
Meredith and Webber try to keep Stan alive but soon they realize that there’s nothing they can do. Anyway to reassure the other man, Ray, who is really worried for his pal, Meredith lies and tells him that they’ll both be ok, but once being extracted from the ambulance he should be operated first. Stan agrees and asks to be able to speak with his wife.
Although still in the grip of excruciating abdominal pains, Shane absolutely doesn’t want to be touched by Bailey and asks insistently to be examined by a male doctor, but, when Webber arrives, he still doesn’t want to be touched and demands the consult of a different doctor. Webber realizes that the man wants a white doctor and, offended, leaves the woman to deal with Shane alone, while he goes to take care of the two paramedics in critical conditions. So Bailey gets Cristina, who is pissed because she’s been pulled off from Dr. Hahn’s team and obliged to examine Shane that, even if reluctantly, let her do her work. During the examination Cristina finds out that he has a giant swastika tattooed on his abdomen. Although disgusted by the Nazis symbol, she tend the man, who tries to justify himself calling his tattoo a ‘ juvenile mistake’ and he tells her that she must take care of him, whether she likes it or not.
Following, the analyses reveal that the man has a big ongoing haemorrhage and needs to have a surgery. Cristina tells about the tattoo to Bailey who, even if very upset by the racist behaviour of the man, chooses to not abandon the patient - although in her own right to do so- to not show signs of weakness and to prove to be superior. So she decides to do the surgery on him but she wants Cristina to be present too.
Shane initially says he has no intention to sign the consent form for the operation, but after his demand to have at least one white doctor in the operating room, he agrees to undergo the surgery.
At the ambulance meanwhile things become complicated, the poor Stan dies after speaking with her wife , Ray loses consciousness and they can’t extract him from the ambulance without before treating the haemorrhage which is compromising his breathing. Meredith decides to try to save him at any cost and crawls into the front seat of the ambulance.
